INSURANCE INFORMATION AND FINANCING PLANS
The Fertility Institutes have a long history of providing the highest quality
infertility treatment services at substantially discounted prices. We always
have, and will continue to match or better ANY published price for a
fertility service that we offer.
FINANCING We offer full financing of fertility treatment
plans on approved credit. This financing allows for a payment plan of
up to 12 months for most of the procedures we carry out. Financing is handled
by financial instiotutions affiliated with our Centers. A special line of
credit is offered credit worthy enrollees allowing your fertility cycle to
be paid in advance, with repayment carried in monthly installments. Please

number, group number and social security number. Include the companies phone
number and your employer's name. Please make your questions brief and to
the point. If you have a "Benefits" page from your policy plan, please fax
this to us (818-776-8754) with your name on it.
Also, it is VERY important to our counselors to receive a faxed copy of your
"EXCLUSIONS" page from your plan booklet. Once we recieve your inquiry,
someone from our staff will reply by phone or E-Mail within a few days.
